INSURANCE CLAIM PROCESS

All players must pay into the league insurance pool, regardless of their insurance situation. In the event of an injury on the field, the team rep must immediately contact president Jen Larson to notify her of the injury. Please make sure the referee logs the injury in the game report as well. If a claim is necessary, here are the steps to take:

INJURY CLAIM PROCEDURE (as of September 2011):

  1. Player downloads the USASA Claim Form from CSSA website, AND NOWHERE ELSE (not USASA, not an old form with Al Bell's name on it, etc.). Please note that the form is a PDF file and you will need Adobe Reader to open and/or print the file.

  2. Player fills out the form completely, EXCEPT the team manager/coach/referee, league and CSSA verification officer signature blocks (Part B, lines 7 and 8, and signature block at very top of page 1).

  3. Player then sends the form to the President Jennifer Larson for witness signature (Part B, line 7).

  4. President signs Part B, line 7 and mails the form to the "designated league verification officer" to have the league verify that the injury is reported in the official USSF match report (referee's report), if applicable. If needed, the league can ask it's assignor or the referee directly to amend the report to include details of the injury (file a supplemental report).

  5. League verification officer signs Part B, line 8 and sends the claim form to the CSSA verification officer, President Dan Rooney, for final sign-off.

  6. CSSA then sends the completed claim form to AIG/Chartis insurance company.
